LipLase from Fotona (Ljubljana, Slovenia) is a fast and comfortable, non-invasive laser treatment revolutionizing lip rejuvenation. “We treat lips from both the inside and outside using LipLase, and obtain immediately visible results,” said dermatologist Natasa Teovska Mitrevska, M.D., M.P.H., of the department of dermatology at ReMedika General Hospital (Skopje, Macedonia).

“Every aspect of the lips is targeted to ensure patients achieve fuller, smoother, tighter lips. In addition, we treat the nasolabial folds for a more youthful look to the mouth,” she elaborated. “Treatment takes only 30 minutes per session, and we typically treat about once a month for three months.”

Using the Er:YAG in gentle and superficial mode, operators target the oral commissures, vermillion border, philtrum ridges and Cupid’s Bow region for an inside-out treatment.

“This provides controlled induction of thermal energy without compromising barrier function in this sensitive area. By increasing tissue temperature to 60° C, we are stimulating collagen production and lifting lips to expose more volume,” Dr. Mitrevska explained.

“We see an immediate effect resulting from shrinkage of collagen fibers. Initiation of neocollagenesis, a process which lasts as long as six months, builds on this effect and leads to overall improvement in elasticity and reduced laxity,” she continued.

Dr. Mitrevska recommends using FotonaSMOOTH® mode with the 7 mm spot size, fluence at 9 J/cm2, and 1.6 Hz, for four to six passes intraorally followed by four extraoral passes.

The second step of the procedure requires the Nd:YAG laser in Fotona PIANO® mode, which delivers ultra-long pulses that penetrate with manageable thermal impact, greatly minimizing damage and reducing downtime. For this second phase, Dr. Mitrevska uses a 15 mm spot and fluence at 90 J/cm2 delivered externally, bringing tissue temperature to 42° C, maintained for three minutes with local cooling.

“This is the safest mode for reaching the deeper skin tissue while sparing structures, such as hair follicles and blood vessels,” Dr. Mitrevska stated.

After treatment, tissue will go through three phases, according to Dr. Mitrevska. “Some edema will manifest and last for about two days, and there may be some surface peeling 24 to 48 hours post-procedure. These are managed with moisturizing lip balm and SPF 50+ sunscreen. The third phase is neocollagenesis.”

LipLase is excellent for the many patients who long for an alternative to injectables.

“Any patient can undergo LipLase, but we must be especially careful with patients under the age of 18 years,” said Dr. Mitrevska.

“I’ve seen the best results in patients older than 40 years of age with a ‘sad’ smile and deep nasolabial folding with wrinkles under the vermillion border,” she indicated. “LipLase smooths out the wrinkles and rejuvenates the youthful appearance of the lips. For younger patients, LipLase can delay the onset of wrinkle formation.”

While LipLase is non-invasive, Dr. Mitrevska says patients must avoid swimming, sports and excessive solar exposure briefly after treatment. And history of recurrent herpetic infection may call for prescription of prophylactic antiviral agents.
