With five wavelengths and intense pulsed light (IPL), the Light EVO from Quanta System (Milan, Italy) does the work of up to six separate devices. Available wavelengths include 532 nm, 755 nm and 1064 nm, as well as optional 1320 nm and 2940 nm. Its applications include hair removal (LHR) on all skin types, vascular and benign pigmented lesions, atrophic acne scars, fine lines and wrinkles.

Board certified dermatologists David Herschthal, M.D., and his daughter Jordana Herschthal, M.D., chose the Light EVO for their Boca Raton, Fla.-based practice because it offers the most capabilities in one platform.

“To my knowledge, it is the only device that has the 532 nm, 755 nm and 1064 nm lasers and IPL in one platform,” said Dr. D. Herschthal. “And we can add erbium if we want to. I don’t believe there is another manufacturer that offers this combination with so many options.”

In addition, since the Light EVO uses no disposables, “it is much more cost-effective,” he added. “Furthermore, we’re only paying one warranty after the initial two years – not five or six different warranties for separate devices.”

The Drs. Herschthal use the Light EVO mainly for pigmented and vascular lesions and LHR. The process begins with determining the patient’s skin type and matching the wavelength to the absorption peak of the intended target. For example, “LHR in skin types I through III requires the 755 nm alexandrite laser, versus the 1064 nm Nd:YAG laser for phototypes IV through VI,” said Dr. J. Herschthal. “The 1064 nm wavelength bypasses the epidermis to treat dark, coarser hair in skin of color.”

“The target absorbs the wavelength,” Dr. D. Herschthal added, “and that is the end of the target.”

The laser also offers a Burst mode featuring up to three tunable pulses. According to Dr. J. Herschthal, it is ideal for resistant hair and veins and lighter pigment.

“In Florida, we see patients who have constant sun exposure. The Burst mode works by increasing heat in both the treatment target and skin, allowing the skin to cool while maintaining heat in the target during the ‘off’ phase,” she explained. “This allows heat to accumulate in the target while protecting the skin from damage.”

With fast repetition rates and spot sizes up to 20 mm (Domino EVO model), the EVO platform also works quickly, even on larger LHR areas such as the back. LHR and IPL facial area treatments (the latter using wavelengths between 400 and 1200 nm) typically take three to six sessions, versus one to two for pigmented and vascular lesions, Dr. D. Herschthal pointed out.

Most patients require no anesthesia. Before treatment, however, the dermatologists offer patients nitrous oxide. “For zero discomfort, we use ice and a Zimmer chiller,” Dr. D. Herschthal noted.

After treatment, patients experience very thin crusting or darkening of target lesions for three to five days, he continued. “But since it is a non-ablative procedure, we tell patients there is no downtime because they can cover these areas with a tinted moisturizer or sunblock. Patients love the results and refer their family and friends.”
